Private Health Insurances and Managed Care Insurances require a prior authorization and/or a pre-certification before the home health agency can provide care.

Occupational Therapy

Our Occupational Therapists will focus on the upper extremities with the goal of developing upper body strength so that our patients will continue to independently perform their day to day tasks requiring the use of both hands.

Medical Social Workers

Our Medical Social Workers will be the bridge to connect our patients to community services and resources that might help assist in their long term recovery.

  • Conduct assessments of the patients social needs to help determine their eligibility for services.
  • Assist patients in accessing community resources.
  • Helping patients apply for government benefits such as Medicaid or Medicare.
  • Provide counseling to patients and their families to help cope with stressors related to illness.
  • Provide emotional support to patients and families during difficult times such as serious illness, accidents or death.

Home Health Aides

Our Home Health Aides will be the helping hands to assist our patients so that they are able to continue to perform most of their personal daily routines and activities during recovery.

  • Assist patients with getting dressed and undressed.
  • Assist patients with grooming and hygiene care.
  • Make the patients bed.
  • Deliver and help patients with bathing or showering.
  • Light housekeeping.
  • Vacuum the patient’s area.
  • Organize and clean the patient’s area.
  • Doing the patients’ laundry and folding clothes.
  • Prepare the patient’s meal.
  • Feed the patient.
  • Provide companionship.

We currently accept:

  3. Private Health Insurance Plans

Home Health Benefits Eligibility:

  1. Patient must be under the care of a Medical Doctor.
  2. Patient is getting services under the Plan of Care established by the Medical Doctor.
  3. A Medical Doctor must certify that the patient needs one or more of the following:
    • Skilled Nursing
    • Physical Therapy
    • Occupational Therapy
    • Speech Therapy
  4. Patient must be Certified as Homebound by the Medical Doctor. Being homebound means:
    • Leaving the home is a taxing effort for the patient.
    • Leaving the home is not recommended due to the patient’s medical condition.
    • The medical condition keeps the patient from leaving the home without the help from another person; the need for special transportation; or the use of assistive devices such as a wheelchair or a walker.

*A patient is still considered homebound if they leave the home for medical treatments or for short & infrequent non-medical reasons such as attending religious services or attending adult day care service.
